- 2 times your annual salary rounded up to next highest thousand
- Maximum of $250,000
- 50% reduction of benefit amount at age 65
- Benefit terminates on the earliest date of termination of employment or age 70
Optional Life Insurance
- Increments of $10,000 to a maximum of $500,000
- Note if you are covered under this plan as both an employee and a spouse, you are limited to $500,000 maximum
- Evidence of Insurability coverage Detail required
- Benefit terminates on the earliest date of termination of employment or age 65

Optional Group Critical Illness
- Coverage for both employees and spouses is available and offered as either a percentage of your salary or in units of $10,0000
- 100, 200, 300, 400, or 500 per cent of your annual salary, up to a maximum of $250,000
- in units of $10,0000 up to a maximum of 250,000
- Evidence of Insurability coverage Detail required
- Benefit terminates on the earliest date of termination of employment or age 65

Evidence of Insurability Forms
If you are interested in applying for Optional Life Insurance and/or Optional Group Critical Illness (Optional Group Critical Illness is not available on grandfathered plans) you need to complete an Evidence of Insurabilty Coverage Application and mail your completed application to Canada Life. Canada Life will inform you by mail if your application was approved.

Dependent Life Insurance
- $5,000 spouse
- $2,000 each dependent child
- Benefit terminates on the earliest date of termination of employment or age 70
Out-of-Country
- Emergency care maximum $5,000,000/lifetime
- Non-Emergency maximum $3,000/3 calendar years
- Maximum trip duration 180 days
